To register your business you will need a completed W-9 with Taxable Reporting Number, Certificate of Insurance with HOA Advisory Group, LLC listed as Additional Insured, Contractor’s License and Contractor’s Bond if applicable.

In order for you to receive payment, you must complete this completed registration form with the required documents.
